Florida PACE Centers (Program of All-Inclusive Care for the Elderly) is a personalized health plan and community-based provider of coordinated healthcare, support services, and social engagement for seniors who are 55+, live in one of our service areas, qualify for a nursing home level of care, and can live safely at home or in the community with support. Our goal is to help our participants stay safe and healthy, living in the place they call home.
Enrolling is Easy
To qualify for enrollment, you must be:
- At least 55 years old.
- Able to meet the nursing home level of care requirements outlined by the Department of Elder Affairs.
- Able to live safely at home or in the community with support.
- Live in one of our service areas.

Personalized to Your Needs
Because everyone’s needs are different, Florida PACE Centers provides an interdisciplinary approach to care and offers a wide array of services. Our experienced team evaluates your health, social, and quality of life needs to develop a customized care plan.
That means personalized attention, coordinated care, no-worry transportation, meals, activities, social engagement, and more — all in a trusted environment.
Our services change along with your needs. Whether that means fewer or more frequent visits to your home or our adult day health center, different medications, additional social support, and more. We are here to help you as you age.
In addition to complete, personalized healthcare and social engagement, we will also:
- Provide transportation.
- Coordinate all health and personal care needs.
- Make appointments.
- Manage prescriptions.
- Provide meals and activities (if desired).
- Communicate openly with you.
- Help you plan for future care needs.
How Much Does it Cost?
If you have Medicaid only or are eligible for both Medicare and Medicaid, you may receive care from Florida PACE Centers at no cost.* If you are eligible only for Medicare, you would pay a monthly premium.
